This plan is not dental insurance, but an affordable alternative. The plan was designed by Dr. Barbell for patients without dental insurance to obtain dental treatment and to maintain a healthy smile at reduced pricing.

ENROLLMENT INCLUDES THE FOLLOWING FOR A 1 YEAR PERIOD

• 2 Prophylaxis (cleanings)
• 2 Periodic (check-up) exams with cancer screenings
• 2 Emergency Exams
• 1 Set of bite-wing x-rays
• 1 Fluoride treatment for children 15 and younger
• 20% discount on all other dental procedures*

PLAN RATES

• $375 – 16 years of age and older / $399 for new patients
  (reg. fee for an adult coming in 2x per year is $565, not including emergency exams… that’s a $190 savings)
• $325 – 15 years of age and younger
  (reg. fee for a child coming in 2x per year is $490… that’s a $150 savings)

Enrollment fees are nonrefundable and nontransferable.

Full payment of fees are due at time of service.

Cannot be combined with insurance or other discounts. If 2 or more appointments are missed/broken with less than 24 hour notice, this will nullify participation in the plan and the enrollment fee will be forfeited.

*Plan excludes orthodontic treatment and teeth whitening treatment/products.
